Canadian Addiction Treatment Centres (CATC) is the largest independent addiction treatment provider in Canada.  Based in Ontario, it operates a network of over 70 treatment centres.  CATC’s Methadone Maintenance Therapy (MMT) program is one of the cornerstones of an all encompassing strategy to address opioid dependence, its consequences and its prevention.

CATC’s best practices approach includes using ECG testing as a means of monitoring patients who have started the MMT program or who have had changes in their medication dosage.   However, CATC does not have the in-house, specialized IT and administrative resources it requires to support and expand an ongoing ECG testing program.  In order to improve efficiency, turnaround time on reports and patient care, CATC requires a specialized, customized ECG solution that includes interfacing to its EMR application.

In order to achieve these objectives, CATC has turned to Telemed Diagnostic Management (TeleMED), a pioneer in supplying Canadian healthcare organizations with all-inclusive, integrated solutions for acquiring, interpreting, sharing and storing non-invasive diagnostic data.  TeleMED has entered into a multi-year agreement with CATC to provide the equipment, software and support services that CATC needs to upgrade, maintain and expand its ECG testing operations.

TeleMED will integrate CATC’s existing ECG devices into its solution.  In other cases, TeleMED  will provide devices to replace legacy devices.  The solution will automatically stream test data from CATC’s testing sites throughout Canada and will manage interpretation of and access to those tests from its cloud-based, vendor-neutral and proprietary application, ViTELflo.

Equally important is that TeleMED will provide CATC with a customized support program that includes expedited replacement and repair of equipment  as required and 24/7/365 Call Centre technical support for end-users.

For CATC, its partnership with TeleMED brings cost-effective sustainability of its ECG testing program and an ECG service that can be scaled upwards as CATC enlarges its operations across Canada.  For TeleMED,  this contract means expansion of its technology and services into treatment and rehabilitation, a vertical that it did not previously serve.

After years of investing in information technology, cardiology and radiology clinics are now generating higher volumes of diagnostic data than ever before.  Understandably, physicians who seek to deliver the best care possible, want easy access to all available data in order to make the best evidence-based diagnostic and treatment decisions.

Sonic Healthcare selects TeleMED for managing cardiology diagnostic data

Sonic Healthcare is a global healthcare company with a reputation for excellence in laboratory medicine/pathology, radiology/diagnostic imaging and primary care medical services.  Headquartered in Australia, Sonic has grown to become one of the world’s leading healthcare providers with operations in Australia, the USA, Germany, Belgium, Switzerland, the United Kingdom, Ireland and New Zealand.

Operations in Sonic’s Australian Pathology Labs include offering ECG, Holter and Ambulatory Blood Pressure diagnostic services at many of its Collection Sites.  Historically, Sonic has used a “single vendor” approach to select the brand of devices that it uses for these services and it currently uses a LAN-based data management solution to manage most of the clinical data that it acquires in rendering these services.   In employing this solution, Sonic has encountered a number of limitations related to interoperability, compatibility, flexibility and scalability.  More and more, the current solution is being viewed as outdated technology.

In order to escape these limitations and to set the stage for growing its cardiology testing services, Sonic has recently entered into a multi-year partnership with Telemed Diagnostic Management Ltd. (TeleMED) to migrate these services to an integrated, enterprise-wide, web-based system.

A critical factor in choosing TeleMED was that its proposed solution offered more integration than Sonic has had with previous vendors who were much more proprietary.  Pre-TeleMED, Sonic’s data management system was limited to managing data from one brand of devices.  The move to TeleMED technology means that Sonic will now be using a vendor-neutral archive that accepts data from devices manufactured from all leading manufacturers.  This change makes it possible to mix and match devices as Sonic finds it advisable.  Furthermore, Sonic will not have to bear the expense of maintaining an archive of legacy data because the migration of that data to TeleMED technology can be accomplished in a simple, time-effective and cost-effective manner.

Limitations in the scalability of Sonic’s pre-TeleMED solution resulted in duplication in network architecture among multiple regions.  However, the easy and limitless scalability of the TeleMED solution means that all test data, from all regions, can be streamed into and maintained within one single, united database.  From an IT perspective, moving to an enterprise solution means that redundant and unnecessary hardware will be eliminated and that there will be much less architecture for Sonic to manage.  From a clinical perspective, historical data that is currently only available from a regional database will become accessible from anywhere within the enterprise.   Data of all modalities, from all collection sites and using all brands of devices, will be available in a single database and will be within reach from a single point of access.

While these IT, operational and clinical benefits are tangible and important, the partnership between TeleMED and Sonic is also expected to deliver significant financial benefits to Sonic.  Restrictions on the scalability of its pre-TeleMED solution presented difficulties in growing its cardiology services within and beyond its two largest business units.  However, the TeleMED solution that is being adopted can accommodate an unlimited number of  collection sites, connected devices, concurrent users and amount of data.  In addition, the ViTELflo platform is so versatile that it can be used for managing a wide variety of cardiology and radiology testing modalities.  Therefore, Sonic will have the IT foundation it needs to grow its cardiology and radiology testing services wherever opportunities exist.

Alberta Innovates, through its Accelerating Innovations into CarE program (AICE), has accepted the funding application made by the University of Alberta and partners that include Telemed Diagnostic Management Inc. (TeleMED).

AIberta Innovates (AI) is an Alberta-funded Corporation tasked with delivering on the research and innovation priorities of the Government of Alberta.  It focuses on the full research and innovation continuum, taking ideas from the lab to the marketplace.

The approved application, titled Implementation of Blood Pressure Telemonitoring  and Case Management after a Cerebrovascular Event,  describes a multi-staged, collaborative initiative between University of Alberta researchers, the Alberta Health Services Strategic Clinical Network and TeleMED.  This first stage will assess if home blood pressure telemonitoring and case management for stroke victims can be implemented in Alberta.  The second stage of the project will be to develop an economic model of home blood pressure telemonitoring in Alberta.  This will be done to confirm that home blood pressure telemonitoring with case management is cost-effective and even cost-saving in Albertans who have had a recent stroke.  In the third stage, a group of stakeholders will be assembled to determine how to implement this intervention in a manner that is sustainable.

TeleMED will contribute to this project by developing a customized home blood pressure telemonitoring system that will operate on ViTELflo, its proprietary platform for management of non-invasive diagnostic data.  It will also provide support for users during the project and will, subsequently, participate in the development of a sustainable funding model for providing this service on a widespread level.

Through their combined efforts, TeleMED and Alberta Health Services deliver fully integrated medical diagnostic tests to thousands through Alberta Netcare –the name for all the projects related to the provincial Electronic Health Record. This ensures tests results and health records are now available to the more than 34,000 physicians, pharmacists and other health service providers who regularly access the Netcare Portal. Since these records are available to registered Alberta Netcare users at the point of care, health service providers can formulate and quickly implement evidence-based diagnostic and therapeutic decisions.

Integrated Auto-Faxing Solution

TELEMED’S INTEGRATED AUTO-FAXING SOLUTION DELIVERS SECURITY, SAVINGS AND PRODUCTIVITY

In 2015, when Vitalim Diagnostic Imaging (Vitalim) opened a state-of-the-art cardiac testing and diagnostic imaging practice just north of Toronto, it engaged Telemed Diagnostic Management Inc. (TeleMED) to design and deploy crucial healthcare technology infrastructure.  As part of that infrastructure, TeleMED’s proprietary ViTELflo application acts as a hub, providing a secure, cloud-based platform for centralization of test data from a variety of modalities and for test interpretation.  ViTELflo also automatically transmits those test results to Vitalim’s EMR.

TeleMED Partners with the University of Alberta for Hypertension in Seniors Trial

Dr. Raj Padwal, of the University of Alberta, and his research team have selected TeleMED Diagnostic Management Inc. (TeleMED) as its Health Technology Partner in a study titled Telemonitoring and Protocolized Case Management for Hypertension in Seniors.  This study is funded by Canadian Institutes of Health Research, the major federal agency for funding health research in Canada, and will be a randomized controlled trial to determine if telemonitoring is the optimal method for controlling blood pressure in Canadian seniors.

According to study details, hypertension is highly prevalent in seniors with diabetes and substantially increases morbidity, mortality and health costs. However, treatment of hypertension in seniors with diabetes is a complex challenge.  The need for aggressive BP control in seniors with diabetes must be balanced against the very real risk of serious treatment-related complications.  Currently, office BP measurements are used to monitor the vast majority of Canadians with hypertension.  However, office measurements require patients to attend office appointments and represents a barrier for seniors with mobility or financial limitations.  Furthermore, measurements are usually taken infrequently (i.e., every few months) and this limits the ability to make timely therapeutic adjustments in the event that the BP is low or high.  As a result, the Canadian Hypertension Education Program Recommendations Task Force (CHEP) recently released a strong endorsement of out-of-office over in-office measurements.  Currently, though, the optimal implementation method for use in Canada, in terms of effectiveness, acceptability and costs, remains unclear.

Over 20 published trials reported clinically important BP reductions using home BP telemonitoring and case management.  No trial in seniors has ever been performed and it is important to confirm feasibility, effectiveness, safety, usability and acceptability in this population specifically, or in Canada more generally.  This study will be a 300 patient randomized trial in community-dwelling seniors that compares three in-home measurement models – measurement without telemonitoring, measurement with telemonitoring alone and measurement with telemonitoring plus case management.

TeleMED will custom-design, engineer, deploy and maintain a web portal for securely storing clinical data, controlling clinician access and displaying data.  When designing the web portal, data interpretation and display are two key components that will be considered.  Relevant data trends will be displayed for ease of interpretation, providing easier decision-making for the clinician. This will include any relevant data calculations such as daily, weekly and monthly BP averages and mean arterial pressure displayed on the graph with systolic and diastolic measurements. Data trends over time are vital; therefore, threshold based alert flags will be incorporated into the template to provide alarms when dangerous values and trends are transmitted by a patient. These thresholds will be set to correspond directly with the Canadian Hypertension Guidelines.
